Introduction
Summary
Hallinan uses a pediatrician, anesthesiologists, farmers, and global warming to set up the premises of his book. Generally, it is that by understanding how both humans and the environment contribute to mistakes we can minimize them.
Discussion
Sounds familiar, doesn't it? So far, this sounds like another angle at something we've been hearing all along. I hope you weren't expecting profound insights on the introduction.

Summary
This chapter mostly consists of providing examples of the kinds of mistakes that are made. Examples include petty things like finding beer and serious things like finding tumors. A cause pointed at is short attention spans.
Discussion
The tumors thing is pretty disturbing...the airport screeners less so. I guess everyone already knows you can get whatever you want on a plane if you try. I hope this guys finds some new points to make and doesn't just read like a rehash or Norman's slips & errors chapter.
